Transitions Homeless Center

The primary goal of Transitions is to move people from homelessness to permanent housing. In five years, since Transitions opened its doors, we have helped over 1,400 individuals transitions out of homelessness and into a place of their own within the community. Another 4500 progressions into positive living situations have taken place.
